The RN Care Case Manager (RN RW IMM) blends the mission with Care Management across both Ryan White and Pharmacy programs. Utilizing strong nursing clinical skills, RW IMM Nurse focuses on a caring and knowledgeable approach. You’ll work with our providers, pharmacy and healthcare center team so that patients receive the support they require and access to available resources. This will include providing education and assistance so that eligibility for the program is understood. Working directly within the Healthcare Center, you will complete Health Risk/Needs Assessments, develop Action/Service/Care Plans, and provide ongoing education to patients so that they understand their diagnoses and their medications. For Ryan White contracts that combine psychosocial needs within the RN MCM, you will also provide services addressing mental health needs and basic needs. By developing relationships with programs and agencies serving those needs, you are able to facilitate referrals, and provide support and assistance for your patients who lack those services. In this way you will support the patient’s ability to better understand their health status so that they can work to achieve their healthcare goals.
By utilizing core principles of nursing, you will meet our patients where they are, identify strengths and opportunities for growth and assist them with realistic goal setting. Advocating for your patient, you will bring the essence of the Ryan White Care Act - the safety net for the uninsured HIV/AIDS patients - to our most vulnerable population. By establishing Health Risk/Needs Assessments, Service/Action/Care Plans, referrals and support will directly contribute to success in meeting the deliverables of our Ryan White contract. Beyond the scope of the contract, your work contributes measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services and URAC (Utilization Review Accreditation Commission).
The Ryan White IMM Nurses often the liaison between providers and provides enhanced pharmacy services. The patient and the Ryan White IMM Nurse work together each step of the way towards health education, understanding, better health outcomes and quality of life.
